The role of Patient Flow Coordinator at Barnsley Hospital NHS Foundation Trust is an exciting opportunity for apprentices studying for NVQ Level 2/3, offering a trainee salary until completion of their apprenticeship. Located within the RightCare department, the coordinator will support patient flow across hospital and community services by managing admissions and discharges from Intermediate Care beds. The position requires an individual who is motivated, organized, and has experience in acute or community care settings, as well as exceptional interpersonal and decision-making skills.

The coordinator will work closely with a diverse team including nurses and social care professionals, processing electronic and telephone referrals to various health and social care providers, and ensuring timely follow-ups to confirm service provision. The ultimate aim of this role is to ensure that patients receive appropriate care at the right time and place, contributing to the efficient operation of the RightCare service and enhancing the quality of care for Barnsley residents.
