A fantastic opportunity has arisen for an exceptional Pharmacist to join our large and friendly pharmacy team at University Hospitals Coventry and Warwickshire NHS Trust. The successful post holder will be part of our highly valued and skilled Oncology, Haematology and Palliative Care team. They will have a chance to make a difference every day and deliver exceptional service.
In undertaking this post you will gain experience and receive training in all aspects of our services for oncology and
haematology
patients. You will rotate between our in-patient wards providing a patient focused clinical service and be trained to support specialist adult and paediatric clinics, clinical trials, clinical verification of chemotherapy, aseptic services and our chemotherapy electronic prescribing system.
The ideal candidate will be ambitious, self-motivated and organised with excellent communication, time management and interpersonal skills. You will be able to work effectively alone and as part of a team and will contribute to the process of service development and innovation by your own initiative and enthusiasm.
The role is offered on a full-time basis. You will be based at University Hospital Coventry, a PFI hospital opened in 2006 which is situated just off the junction of the M6 and M69.

We are a large Acute Teaching Hospital of approximately 1300 beds. Our pharmacy department has an innovative approach to pharmaceutical care with a ward-based technician service and pharmacist led clinics. We have strong links with the Medical School at Warwick University and the department of Medicines Management at Keele University.
The post provides an opportunity to develop clinical and technical skills within a large acute teaching hospital. There are also developmental opportunities through contributing to a speciality group; governance, guidance, budget and local policy.
Successful candidates will work in our busy MHRA licensed aseptic suite within an excellent and highly experienced team of specialist pharmacy technicians, assistants and pharmacists. You will also support the provision of chemotherapy to our chemotherapy day unit at our sister hospital, The Hospital of St Cross in Rugby.
You will provide a clinical pharmacy ward service and be trained to clinically screen chemotherapy prescriptions as part of your role.
You will also be able to develop your palliative care knowledge and may have the opportunity to provide pharmaceutical care to our local hospices.
This position requires you to have a postgraduate Certificate in Clinical Pharmacy (Hospital) or equivalent, possess excellent communication skills and be able to manage your time effectively.
The department supports CPD for all staff.

- Check prescriptions for clarity, safety and efficacy on wards, in dispensaries and the aseptic suite and when necessary take accurate drug histories to facilitate this. Query with, and provide the prescriber with appropriate information to effect a change in the prescription when clinically required, to ensure patients safety and effective therapy. In complex situations seeking the advice and support of a senior pharmacist.
- Provide a clinical pharmacy service to patients in the Trust following the departmental and Trust policies and procedures. These will include working to a framework of medicines management and clinical governance including the RPS hospital pharmacy standards.
- Provide information to nursing and medical staff on allocated wards on any aspect of medicines policy ensuring safe practice. Provide information to patients on allocated wards or in dispensaries concerning their medication and answer any resulting questions or queries.
- Provide when necessary, through checking dispensing or supervision of technicians, an efficient medication supply service from any of the pharmacy departments, including the aseptic suite.
- To work with the Senior Pharmacist(s) for the clinical areas to ensure effective medicines optimisation and management. This may include being responsible for audit work, attending meetings and education and training of ward staff. It will also involve ensuring the effective leadership and management of floor based teams, deputising for the Senior Pharmacist(s) as needed.
- To work with the Senior Pharmacist(s) for the clinical areas to develop policies, guidelines, audits, reports and research relating to the use of medicines within their area(s).
- Ensure patients discharge medication is accurate and dispensed in advance of the patients discharge, by writing the patients medication on the GP discharge prescription and conveying to the GP all necessary information concerning the patient medication to ensure a safe transfer to primary care. Reducing pharmaceutical waste by utilising patients own drugs in the discharge process
- Within your designated clinical area of responsibility, ensure best practice and clinical governance is applied to drug therapy by providing medical and nursing staff with:-
- Appropriate evaluated drug information with recommendations for patient treatment
- Support in writing clinical guidelines and shared care guidelines
- Participation in and delivery of education and training
- Undertaking clinical audit and practice research as required and supervised by Senior Pharmacy Staff.
- Ensuring procedures for the introduction and monitoring of new drugs are followed
- Monitoring of drug expenditure and provision of advice on the financial implications of changes in practice
- Train, and act as role model to pharmacy staff allocated to specialty
- Liaise with community medical nursing and pharmacy staff to ensure effective and seamless care
- Leading on Medicines Management and Medicines Optimisation Issues
9.Provide clinical pharmacy and prescribing services within designated areas, if needed.
10. Work flexibly across all of the Pharmacy departments, covering the work of colleagues during peak periods or absence.
11.Participate in appropriate training and development activities including your own personal development and participate in the education and training of colleagues and all other healthcare professionals on medicine legislation, pharmacology, medication risk and best prescribing practice
12.Participate in weekend and Bank Holiday working as may be required. This may require working as the sole professional practitioner and will require response to unpredictable solutions
13.Undertake any other duties as required by the Director of Pharmacy UHCW in accordance with the grade and nature of the post.
