The Patient Pathway Coordinator role at Harrogate and District NHS Foundation Trust entails delivering efficient administration services for outpatient bookings and patient pathway management within the Long Term and Unscheduled Care Directorate. This role requires excellent interpersonal skills for effective communication with patients and staff across various levels of the Trust, while also building relationships with clinicians and other administrative teams. Additional tasks include managing clinic schedules, supporting the Lead Patient Pathway Coordinator, training staff, and providing coverage for the Lead Coordinator during absences. Overall, this position is vital in ensuring high-quality support for patients and efficient outpatient service delivery.

We are looking for a Patient Pathway Coordinator who can help us deliver an efficient, high quality medical appointments service across our Long Term and Unscheduled Care Directorate.
The successful candidate will:
- provide efficient and effective administration services for outpatient bookings and patient pathway management.
- prioritise their own workload to ensure registration of all referrals and booking of appointments are in line with the Trust's waiting time standards and the Trust's Patient Access Policy
- work to defined working procedures to maximise effective use of clinic capacity and help ensure the Trust meets local and national waiting time targets.
- accurately record information on to systems and maintain patient appointment information.
This role provides a great insight into the NHS and requires excellent inter-personal skills when communicating with patients and members of staff at all levels within the Trust.
The Patient Pathway Team is located in the Trust's Long Term and Unscheduled Care Directorate . They work closely with the management team to deliver an effective and efficient Outpatient Service ensuring a high level of support for patients.
The teams responsibilities includes:
- Management of appointments and referrals, supporting clinical teams through the triage process and booking outpatient appointments.
- Understanding clinic templates and monitoring utilisation.
- Monitoring waiting list and carrying out validation.
- Patient communication via letter and telephone.
- Building relationships with clinicians, nurses, managers and other admin teams.

Main responsibilities:
- Clinic Creation/Changes
- Management of clinics which includes clinic cancellations, reappointment of cancelled appointments and checking availability of clinic slots.
- Supporting the lead Patient Pathway Coordinator to monitor capacity and manage both new and follow up waiting lists.
- Actioning outcomes from clinics, including making follow up appointments.
- Training staff.
- Providing annual leave cover for the Lead Patient Pathway Coordinator.
