The Telford Community Staff Nurse position with Shropshire Community Health NHS Trust is an opportunity for a dedicated Registered Nurse to join the North Telford District Nursing Team, focusing on Planned and Admission Avoidance care for patients in the community. Candidates should be committed to interdisciplinary teamwork and delivering high standards of care, with a strong enthusiasm for community nursing and a willingness to pursue ongoing professional development. The role requires independent travel, and offers the chance to assess, plan, implement, and evaluate nursing interventions while collaborating with colleagues to ensure comprehensive patient care.
The successful candidate will work within a dynamic team, providing clinical support in various settings and ensuring legal frameworks and consent protocols are upheld. Responsibilities include maintaining patient records, reporting on clinical findings, and educating patients and families on therapeutic activities. The position operates on a rota system covering 7 days a week, from 8 am to 6 pm, and emphasises a commitment to continuous professional development and training. An exciting opportunity has arisen for a highly motivated Registered Nurse to join our Community Nursing Team within the North Telford District Nursing Team in the provision of Planned and Admission Avoidance care to patients in the community.
We are looking for a caring, motivated friendly community nurse to join our busy, professional and dynamic team. You should possess commitment to interdisciplinary team working and innovative delivery of excellent standards of care. You will be able to demonstrate a real enthusiasm for community nursing, as well as a desire for on-going professional development.
Ability to travel independently to fulfil the requirements of the post is essential.

- Promotes a 'can do' attitude and determination to develop and sustain a dynamic and responsive administrative and operational service within a community setting which delivers best practice.
- Able to assess, plan, implement and evaluate interventions, liaising with colleagues and partners to ensure an integrated approach to the needs of the service user.
- To organise and deliver skilled clinical support work without direct supervision. This will include individual in a variety of settings.
- Gain valid informed consent and work within legal frameworks with patients.
- To liaise with senior clinicians with regard to patient care, highlighting any variations to expected outcomes from agreed treatment programmes.
- To provide basic verbal and written clinical reports and document patient interventions/findings in case notes in line with professional body and Trust standards.
- To instruct and educate patients, carers and families on a range of therapeutic activities.

The post is covering 7 days 8am to 6pm service, on a Rota System.
