The IHDT Nurse position at Livewell Southwest in Plymouth involves supporting the Complex Discharge Team with a focus on empowering patients to continue their care at home. This role, which requires 37.5 hours per week, emphasises strong leadership, communication, and interpersonal skills, paired with a systematic and composed approach to work. The nurse will be responsible for conducting clinical assessments, coordinating care packages, and collaborating closely with team managers to ensure high-quality, evidence-based discharge planning. The position also involves managing a caseload, mentoring new staff, and participating in policy development and quality assurance initiatives.

Ideal candidates for this role should demonstrate empathy, teamwork, and a commitment to achieving positive patient outcomes.
