HCPC MULTI-DISCIPLINARY NHS SOCIAL WORK
Swansea Bay University Health Board is seeking a motivated and enthusiastic HCPC registered Occupational Therapist to join the Neath Port Talbot Community Reablement Team. The role involves working within a multidisciplinary team to promote independence and enhance the daily lives of patients in the community, including providing rehabilitation services in various settings such as reablement wards, patient homes, and care facilities. The team focuses on helping individuals return home from hospital through the Home First initiative, requiring flexibility and participation in 7-day working.

Candidates should have significant experience in rehabilitation or reablement, preferably within NHS or social care settings, and must possess good IT skills for electronic patient record management. The ability to travel independently between sites is essential, and the Welsh language is desirable. The role also includes providing supervision to less experienced staff and students, with support from senior therapists and clinical team leads.
