The Community Practitioner role at the Cumbria, Northumberland, Tyne and Wear (CNTW) NHS Foundation Trust offers an exciting opportunity for Registered Nurses, Social Workers, or Occupational Therapists to join the expanding Universal Crisis Team (UCT) in Morpeth, covering Northumberland and North Tyneside. Successful candidates will work within the Working Age Adult Pathway, providing urgent triage, home-based treatment, and support for mental health crises. The position requires flexibility and collaboration within a supportive multidisciplinary team, focusing on delivering compassionate and effective care while fostering professional growth and service development.

Candidates should possess clinical experience in managing mental health complexities and risks, and they are expected to engage in a recovery-based approach.
