The Rehabilitation Support Worker role at the Surrey Downs Health & Care in Dorking involves providing care and support to patients in a 28-bed rehabilitation unit. The unit caters to individuals living with frailty, complex needs, and rehabilitation requirements, and operates with a multidisciplinary team to facilitate patient transitions back home. The support worker will execute a variety of delegated tasks as outlined in care plans, report to lead therapists or nurses, and collaborate closely with the entire healthcare team to promote patient independence and effective rehabilitation.

Key responsibilities include delivering clinical and therapeutic care, performing patient assessments under supervision, ensuring safe practices, and maintaining clear communication with patients and the broader healthcare team. The role also emphasises the development of skills and competencies to improve care delivery while advocating for patients' privacy, dignity, and individual preferences.
