The Rotherham Doncaster and South Humber NHS Foundation Trust is seeking a Band 6 Care Home Liaison Nurse for a 12-month fixed-term position in Scunthorpe, due to maternity leave. This role involves collaborating with Care Home staff to offer specialized support, education, and interventions aimed at enhancing the quality of care delivered in these facilities. The selected candidate will be responsible for preventing delays in patient care, promoting nursing standards based on best practices, and conducting clinical audits and effectiveness evaluations. The position also requires assisting with effective patient discharge processes and advising Care Homes on relevant mental health legislation.

Due to maternity leave, there is an opportunity to work into the North Lincolnshire Care Home Liaison team for a 12 month fixed term / secondment.

We are looking for an enthusiastic and motivated Band 6 Care Home Liaison Nurse who wants to participate and contribute to achieving excellent standards of quality care within a Care Home environment. The applicant will work alongside the Care Home to provide specialist support, advice, education and support interventions to the Care Home staff.

To provide specialist support and advice to referring Care Homes, preventing unnecessary delays.
To promote and assist in the maintenance of the highest attainable standard of nursing care in a Care Home setting, drawing on examples of current best practice.
To deliver education, support interventions, and implement processes with consequent improved patient outcomes, therefore supporting the achievements of Rotherham Doncaster and South Humber NHS Foundation Trust and Rotherham Foundation Trust internal performance indicators.
To undertake clinical effectiveness and audit activities throughout the year.
To promote and assist in effective ward discharge and placement preparation for both the patient and their family/carer.
To ensure Mental Health Act and Legislation advice is provided to Care Homes as required.
