Job Summary
The Practice Nurse / Nursing Associate will be expected to provide care in the treatment room for the patients of Tunstall Primary Care Centre and Packmoor Medical Centre. This would normally be carried out by face-to-face consultations including, travel and childhood immunisations, Immunisations, dressings, chronic disease management, contraception checks, Cervical Screening & core nursing services.
Areas of Clinical Responsibility
To carry out nursing care plans and treatments in conjunction with GPs or independently to agreed protocols.
To work in accordance with clinical guidelines and practice policies
To carry out health screening as required
To carry out all general treatment room duties including, dressings,
To carry out immunisations, including travel & childhood immunizations
Undertake NHS health checks and participate in research projects, as & when appropriate.
To undertake Chronic disease review for patients with a long-term condition
Contraception checks Ie- Pill Checks.
To undertake ECGS, Venipuncture where necessary
To undertake holistic assessments for the FAAs (Facilitating Admissions Avoidance)
To perform Cervical Smears for the cervical screening programme.
To maintain appropriate records and statistics
To communicate any abnormal findings to other members of the nursing team and/or the GPs, as appropriate
To visit patients at home as and when appropriate
To be active in health promotion and patient education
To carry out specialist nursing activities as appropriate and to participate in training and development.
To work within the principles of the Practices GMS contract. and Quality & Outcomes Framework (QOF), Quality Improvement Framework (QIF) the LES/DES and the GMS contract
Attending primaryhealthcare team meetings and other meetings as may be necessary for your duties.
Willing to upskill as part of (PDP) Personal Development Plan

We normally arrange for all new staff members to have a mentor or supporter within the practice team.
