Sussex Community NHS Foundation Trust (SCFT) are excited to be recruiting an Operational Support Officer for the Transfer of Care Hub, based at the Royal Sussex County Hospital. The role will provide business and administrative support to the Transfer of Care Hub (TocH) and their team members.

We are looking for an enthusiastic and motivated individual who will support the Transfer of Care Hub lead on the daily escalation calls, produce reports/data as required and maintain and coordinate the dashboard for the discharge hub, collating updates, actions and comments from the multidisciplinary team (MDT).

About us

The post holder will work as part of a dynamic and fast-paced multidisciplinary team. They will need to demonstrate tact, discretion, and initiative to support acute to community discharge planning.
