The role of Homefirst Assessor - Physiotherapist/OT/Nurse Assessor with Cornwall Partnership NHS Foundation Trust in Helston involves working within a multidisciplinary team to provide integrated community care for the population in West Cornwall. Candidates should be experienced Physiotherapists, Occupational Therapists, or Nurse Assessors with a background in community work. Responsibilities include assessing patients, planning and implementing care strategies, and collaborating with various healthcare organisations to meet the needs of older adults.

The position requires strong organisational skills and knowledge of older person's services, as well as the ability to triage and create evidence-based treatment plans. The role entails regular weekend work within an 8am-6pm schedule, and candidates must possess a valid UK driving license and access to a vehicle for work purposes.
