The Jessie May Care Team Leader role at University Hospitals Bristol and Weston NHS Foundation Trust involves overseeing community-based palliative nursing care for families of children and young people with life-limiting and life-threatening conditions across a specified geographical area, including South Gloucestershire, Bristol, and surrounding regions. The position requires the leader to manage daily care team activities, support Jessie May nurses, and provide holistic nursing care and guidance to patients and their families.

The successful candidate will be responsible for planning, implementing, and evaluating individualized care plans to maintain high care standards while demonstrating excellent communication skills and fostering strong relationships with families and various stakeholders, including healthcare teams and social services. The role emphasises a coordinated, multi-disciplinary approach to enhance the well-being of the children and young people receiving care, alongside ensuring effective service delivery throughout the Jessie May caseload.
