Jessie May delivers community based palliative nursing care to families caring for children and young people with life-limiting, and life-threatening conditions, across various locations. The areas that Jessie May covers includes South Gloucestershire, Bristol, North Somerset, BANES, Swindon and Wiltshire. The service delivery hours are 08:00-20:00, including weekend, bank holiday and overnight on call as part of the 24/7 end of Life care provision.
The post holder will take delegated responsibility for a specific geographical area, leading and coordinating the day-to- day activities of the Care Team within this area. On occasions they will serve as the Duty Lead for the entire service, providing support to Jessie May nurses across all geographical regions. Additionally, the post holder will deliver skilled, holistic nursing care and guidance to children and young people with complex medical needs and support their families. They will also be responsible for planning, implementing, and the evaluating individualised care to ensure the highest standards of care are met.
The post holder will possess excellent communication skills and demonstrate the ability to build and maintain strong, supportive relationships with the children and families on the caseload. They will work collaboratively and effective with a wide range of stakeholders, including healthcare staff, primary care teams, social services, educational institutions, and other relevant professionals across the disciplines. Their role will be pivotal in ensuring a coordinated, multi-disciplinary approach to care, enhancing the wellbeing of the children and young people under their responsibility, while ensuring seamless service delivery across the Jessie May caseload.
