The Newcastle Learning Disability Team is offering a Band 6 Community Practitioner position focused on their positive behavior support pathway. This role entails working within a multidisciplinary community team dedicated to providing high-quality care to individuals with complex needs across Newcastle. The successful candidate will be responsible for managing a caseload of patients, conducting assessments, developing individualized care plans, and collaborating with various agencies to ensure a comprehensive support system. Emphasis is placed on delivering compassionate, evidence-based interventions while promoting recovery, wellbeing, and independence among patients.
