The Community Healthcare Assistant role with Norfolk Community Health and Care NHS Trust in Attleborough involves supporting community nursing teams by visiting patients in care homes to address various care needs such as diabetes management, wound care, and catheter care. The position requires the ability to demonstrate flexibility and a strong understanding of community healthcare principles. The assistant will work predominantly independently under supervision, document patient visits, and communicate effectively with patients and healthcare professionals, maintaining a focus on quality of life and independence for patients with long-term conditions.

Additionally, the role includes participating in health promotion activities, engaging in training and development opportunities, and adhering to infection control policies while managing health records and liaising with other healthcare professionals.
