The States of Guernsey are looking for an Adult Social Worker to join the team at Adult Community Services.

The main purpose of the post will be to provide a high quality research based casework service to service users. The post holder will be based in the Integrated Adult Community Care Team. They will provide consultation, support, specialist advice and joint working within the integrated services to deliver person centred care and support. They will be based within an identified team, acting in an advisory capacity to members of the multidisciplinary team on social care issues relating to adults, educating or acting as a mentor as appropriate. They will also provide training, consultation and supervision as delegated by the Senior Practitioner for less experienced social workers, support workers and student social workers.

To provide a social work service to adults living in the Bailiwick of Guernsey. To carry out assessments using the agreed standardised format. To participate in co-ordinating services for people wishing to remain within their own homes. To offer support to people who may be moving into care.

To work in a flexible way, adapting the service to meet the changing needs of service users from a wide range of social and cultural backgrounds.

To work in an identified social work team within the Integrated Adult Community Care Service.

To form relationships with service users to formulate an accurate assessment of complex psychosocial needs of service users, their families and service providers and assess for services in line with local policy.

To contribute towards a holistic assessment within the multi-professional specialist team, recognising that diversity, values and human rights may influence the decisions of people who use the service.

To provide complex psychosocial support for service users with terminal illness/life threatening illnesses and their families, advocating for service users, carers, and families around complex emotional & practical needs.

To provide specialist social work advice within the Adult Community Health & Social Care Team.

About us

Clinical:

To take a key role in responding to referrals to the Adult Social Work team carrying a caseload of adults in need.

To respect an individuals independence and dignity and to offer person-centred support in making major life decisions and choices.

To offer support, information & signposting to service users, families and carers on matters such as housing, benefits and finances, respite care and other community services, care home placements & third/private sector services.

Assess service users, carers, and familys psychosocial concerns taking into account cultural, spiritual, psychological, social and financial issues and risk evaluation, in partnership with other members of the multi-professional team as appropriate.

Assess familys emotional resilience, coping skills, strengths, and difficulties within this practise setting, using expert social work skills & discussion with the multi-professional team.

Formulate appropriate interventions to meet service user needs in conjunction and liaison with the multi-professional team, the persons personal networks and other community resources.

Undertake person centred assessments, evaluating & reviewing service user and families needs regularly and readjust care plans if necessary.

Comply with Safeguarding Vulnerable Adults Policy, providing advice to the team regarding adults at risk, and make referrals as necessary regarding child protection & adult at risk.

Be aware of risk as it may relate to service users, carers, children and the safety of health & social care workers.

Take responsibility for recognising and addressing the needs of those with mental health problems, learning disabilities, and other vulnerabilities.

Produce reports in relation to Long Term Care Benefit and present to the Needs Assessment Panel in an accurate and timely manner, using fast track where appropriate.

Identify the need for planned respite for service users in conjunction with other members of the community team.

Offer psychosocial consultation to professionals and informal carers involved in caring for people who are distressed, their families, and carers.

Provide emotional and psychosocial support to service users and their relatives, using individual, group, and family techniques with an aim to improve communication within the family/professional network and help to resolve difficulties. Referring those in need of formalised counselling to the psychological therapies team.

Manage an individual caseload, organising and prioritise the workload effectively.

Communication:

Work closely with the Senior Practitioner and members of the multi-professional care teams to construct a multi-faceted assessment of service user and advise health colleagues regarding psychosocial aspects of care.

To participate in weekly Multidisciplinary team (MDT) meetings and condition specific MDT/case conference meetings as necessary.

Attend and contribute to formal multi-professional meetings to review and plan all aspects of service users care as necessary, chairing as appropriate.

Attend and facilitate meetings for the core and wider multi-disciplinary team to discuss complex cases, practise issues and innovative work.

Network with local community resources to advocate for care offered to service users and their families, including health and mental health agencies, hospital, home care, nursing homes, child welfare agencies, substance misuse programs, and benefits offices.

Attend regular social work team meetings to discuss administrative and operational functions of the service, developments in the wider organisation and beyond and participate in exploration of practice issues.

Maintain a close working alliance with the welfare benefits team, discussing and sharing work as appropriate, and meeting formally on a regular basis.

Management:

To develop effective links with families and carers (non-statutory and statutory) in order to achieve a comprehensive assessment and to develop an appropriate care plan based on individual need.

To demonstrate a high standard of practice under their own direction, that reflects a value based approach to working with adults and families that is person-centred.

To be familiar with local community services and their criteria and costs.

To be familiar with Guernseys individual benefit systems and to keep up to date with any changes for the benefit of service users.

To be familiar with, and execute, Health and Social Care Department Policies, including the Safeguarding Adults Policy.

To be aware of health and safety issues in their area of work and report any concerns to Line Managers.

To comply with the principles contained in the Bailiwick of Guernsey Data Protection Law (2concerning improper disclosure, misuse or breach of confidentiality in respect of information held on computer systems or elsewhere.

Training & Development:

To participate in the annual performance development review system.

To actively promote an understanding of the role of Social Worker within the team and to outside agencies.

Lead by example in the multi-professional teams when working with complex families in order to up skill others (e.g. in delivering significant news).

Take responsibility to inform and raise awareness of key issues involved in working with people from diverse ethnic, cultural, and socio-economic backgrounds and highlight good practise in anti-discriminatory practise.

To maintain up to date knowledge of research relevant to practice and contribute to its implementation.

Participate in training/education programmes regarding care of adults and the role of the Social Worker for professionals both within the organisation, and externally.

Undertake training, and attend and participate in education events, such as conference to further individual professional development, keep updated regarding research and practice, and contribute to the development of the service and the organisation.

Attend mandatory in-house training and externally attend mandatory local safeguarding children & vulnerable adults training and keep updated regarding local policies and procedures.

Participate in the induction and orientation of new staff and students on placement from all disciplines.

Work collaboratively as a team to provide support, mentoring, supervision and appropriate work experience for social work students on practice placements.

Participate in regular clinical and managerial supervision.

Take an active part in the internal clinical supervision of other registered and unregistered practitioners.

Take responsibility for keeping updated on current social work practise issues in statutory services and care of adults accessing training when necessary.

Participate in outcomes measurement, research studies, and internal audits in agreement with the Service Manager and the service research priorities.

Act within the legal and statutory professional codes of practice relating to Social Work as prescribed by Social Work England Registration Body.

Administration:

Compose, word process and distribute by e-mail reports, letters and notes from meetings as necessary.

Update the TRAKcare database on a daily basis.

Maintain up to date, concise and legible case notes regarding any casework.

Maintain statistics, and produce as required.
