The States of Guernsey is seeking an Adult Social Worker to join the Integrated Adult Community Care Team, responsible for delivering high-quality, research-based casework services to adults in need. The role involves conducting assessments, providing specialized social work advice, and supporting service users and their families through complex psychosocial situations, including terminal illness. The post holder will collaborate within a multidisciplinary team, offering consultation and mentorship to junior staff and participating in care planning to ensure person-centered support aligns with local policies and practices.

In addition to case management, the Adult Social Worker will engage in training and professional development initiatives, contribute to safeguarding protocols, and maintain comprehensive case records. Effective communication and collaboration with various healthcare and community resources are essential to advocate for service users. The role requires an understanding of cultural and individual differences in needs and values, emphasizing the importance of providing holistic, respectful, and dignified care.
