Band 5: £29,970 to £36,483 a year Per Annum  PERMANENT 
Following an initial induction and ongoing clinical supervision, you will be responsible for: Assessment, planning, implementation and evaluation of care, without direct supervision, to patients, carers and relatives within their home environment.


Successful applicants will be invited to attend an Selection Day at Carew House, Bodmin on 15th November 2024. This will be a morning or afternoon to be confirmed.

You will be part of our Liskeard-Looe Community Nursing Team and you will be working in the beautiful Liskeard-Looe area which encompasses both Bodmin Moor and Seaside resorts. We are looking for a caring, motivated and dynamic community nurse to join our large, busy, professional team. We cover several GP practices within the local PCN and surrounding area. We want you to enjoy the experience that community nursing offers and how empowering people in their own home feels so very rewarding. Your role will require close collaboration with GP's, health and social care colleagues including therapists and voluntary organisations. We would like you to possess commitment to team working and embrace innovative delivery of excellent standards of care. You will need to have a real enthusiasm for community nursing as an autonomous practitioner and a desire for ongoing professional development.

You should also have an understanding of the changing climate of care delivery in the community.

The ability to travel independently across the West Locality to support other teams and to fulfil the requirements of the post is essential, therefore a valid driving licence will be required to enable you to undertake the role of a community nurse.

Moving to work with the team in most need on a daily, weekly or monthly basis as service provision demands.
