An exciting opportunity has arisen to join the Ageing Well team within Chesterfield & Dronfield Primary Care Network.
We are keen to receive applications from candidates who have relevant experience and skills from other sectors as well as health care.
We are looking for individuals who are keen to contribute to our care coordinator service across our network, and impact on the health and wellbeing of our patient population!
Please note working days are Monday-Friday and the services are open 8.00-6.00pm so the successful applicant must be able to work between these hours. The post is for 25 hours per week, specific hours will be discussed at interview.

Job summary:

The role will directly support both the clinical and non-clinical members of the Ageing Well Team in the organisation of patients care.
KEY DUTIES AND RESPONSIBILITIES
Be a first point of contact for general advice and support for care homes with administrative queries, directing them to other members of the team or booking them with the most relevant clinician to support any clinical needs as required.

INFECTION CONTROL
All staff have a duty to comply with Infection Prevention and Control. You have a duty to ensure that you minimise the risk of infections and infectious diseases. The responsibility includes minimising the risks by participating in training, highlighting any concerns you may have to the appropriate person as identified in the policies and procedures.
HEALTH & SAFETY
All staff have a duty to ensure the health & safety of themselves and others whilst at work. Safe working practices and health and safety precautions are a legal requirement. All accidents must be reported to your line manager. You must participate in accident prevention by reporting hazards and following relevant policies and procedures and by participating in training.
RISK MANAGEMENT
All staff are required to contribute to the control of risk and use the incident reporting system to alert the PCN of incidents or near misses that may compromise the quality of services.
CONFIDENTIALITY
All staff working for Chesterfield & Dronfield PCN have both a common law duty and a statutory duty of confidentiality to protect patient information and only use it for the purposes for which it is intended. The disclosure and use of confidential patient information needs to be both lawful and ethical.
