We are looking for an enthusiastic and experienced peritoneal dialysis nurse to join our expanding team. The role will be based in the community, as this is a home-based therapy. You will be supporting our patients in the comfort of their own homes, dealing with issues they may experience due to their dialysis. We will ensure you are equipped to train patients at home, to undertake post training follow ups and support our more vulnerable patients that require regular visits. There may be times where you will be required to work inhouse, to support the team with clinics which we run 4 times a week alongside our consultants. You will carry out routine procedures such as, exit site dressings, PET's, adequacies and line changes as per unit protocol. You will also assess patients' suitability for Peritoneal Dialysis as their choice of therapy.
To have responsibility for the following aspects of care and management:
The post holder has delegated continuing responsibility for the management of the clinical areas in the CAPD unit.
They will provide clinical leadership to the nursing team in the absence of the Unit Manager.
They will lead the nursing team in the provision of safe and effective clinical practice and enhance the patient's experience.
The post holder will ensure continuity, sharing of ideas and the settings of standards of care.
Information and feedback will be shared to empower patients and members of the multidisciplinary team.
The post holder must possess fundamental management and facilitation skill in order to coordinate and supervise the activity/management of the staff, promote multidisciplinary working and foster a supportive learning environment in the absence of the manager.
