Zetland Medical Practice in Redcar is seeking a Clinical Pharmacist to join its multidisciplinary team for a part-time role of 23 hours per week. The successful candidate will play a key role in managing repeat prescription processes, overseeing the safe prescribing of medications, and conducting clinical medication reviews for patients with diverse conditions, including long-term illnesses. The position involves direct patient interaction, working collaboratively with GPs and other healthcare professionals to optimize medication management, enhance patient adherence, and contribute to quality improvement initiatives in prescribing practices.

The role includes comprehensive training through the accredited 18-month Primary Care Pharmacy Education Pathway, with support for obtaining an Independent Prescriber Qualification. Responsibilities encompass patient-facing clinics, managing care for those in care homes or receiving domiciliary care, and ensuring smooth medication transitions during hospital discharges. The pharmacist will also provide telephone support for medication inquiries, participate in collaborative services development, and adhere to compliance with health regulation standards, all while fostering strong working relationships within the Primary Care Network.
