The role of Continuing Health Care Senior Clinical Assessor/Practitioner with Hampshire and Isle of Wight ICB involves conducting comprehensive assessments to determine eligibility for NHS Continuing Healthcare (CHC) and case managing clients to ensure their health needs are met. The position requires a Registered Nurse or Allied Health Professional who is passionate about patient care and improvement, with strong clinical assessment and communication skills. The role promotes individualized care solutions such as Personal Health Budgets, aiming to empower individuals with complex health needs.
