The role of Parkinson's Disease Specialist Practitioner at Homerton Healthcare NHS Foundation Trust is a 12-month fixed-term contract focused on enhancing specialist skills in the care of patients with Parkinson's Disease (PD) across acute and community settings in City and Hackney. The practitioner will provide comprehensive services, including patient and carer support, medication management, and access to therapy and support services. Suitable candidates include healthcare professionals such as physiotherapists, occupational therapists, and nurses, with essential experience in Parkinson's care, although specialist knowledge is not mandatory.

The postholder will operate autonomously, handling direct referrals and managing their caseload independently while collaborating with various teams to promote an integrated approach to PD care. Key responsibilities involve developing treatment coordination, evaluating patient management strategies, and establishing efficient referral pathways to minimize unnecessary hospital visits. The role also includes working within the Homerton Movement Disorders clinic to enhance service quality and access for patients throughout their disease trajectory.
