Kite Practitioner with Betsi Cadwaladr University Health Board in Abergele
Are you a creative and dynamic OT, social worker, Mental Health, Learning disability or children's nurse with experience of working with children and adolescents in a mental health setting? The Kite Intensive Community Support Team provides an intensive service to young people aged 12yrs to 18yrs across North Wales who are experiencing difficulties with their mental health. The Kite team is part of the integrated Tier 4 CAMHS service for North Wales and will work alongside Community CAMHS and the Kestrel inpatient ward to provide an alternative to admission for young people in North Wales and North Powys. As a member of the team you will work alongside community specialist CAMHS colleagues with young people and families/carers to provide specialist child and adolescent mental health assessments, formulations, treatments and short-term interventions for young people experiencing acute and complex mental health problems. Successful candidates should be highly motivated to improve the mental health trajectory of the young people we care for. As a member of the Kite team, you will have the opportunity to work within a small and committed team, where you will have the opportunity to build upon your existing skills and to work in a fast paced and dynamic team. Responsible for the assessment of the patients nursing care needs, and the development, implementation and evaluation of programmes of care to meet those nursing needs. Provide a gate keeping service to the in patient service and out of area beds. Ensure that effective communication is established and maintained with patients and carers/relatives Ensure patient needs are met by working collaboratively with other professionals and agencies, especially in relation to ongoing care needs and discharge arrangements Provide specialist assessments and treatment packages and contribute to risk assessment and care and treatment planning in line with the Mental Health (Wales) Measure. Collect, collate, evaluate and report information, maintaining accurate patient records. Contribute to the development of local nursing guidelines, procedures policies etc. Recognise and respond appropriately to urgent and emergency situations Deliver high quality, evidenced based clinical care.
