We are looking for an Advanced Nurse Practitioner to join our friendly experienced team at our Sparcells site. The role will involve working independently as an autonomous practitioner providing clinical assessment, diagnosis and treatment for our adult and paediatric patients with undiagnosed and existing conditions. You will be working alongside our clinical team of GPs, Nurses, Pharmacists, Physiotherapists, Health Care Assistants and other associated roles to provide safe, holistic care to our patients. This will include face to face and remote appointments. You will be required to work within your professional competence and boundaries to provide both urgent and planned care to patients.

The post holder will work autonomously to provide comprehensive healthcare that is sensitive to the needs of the population. They will take a leading role in managing the clinical services provided by the practice, ensuring that they meet the needs of our patients and fulfill the terms of our contracts.
SPECIFIC RESPONSIBILITIES
Clinical
See patients that present with previously undifferentiated, undiagnosed problems, taking responsibility for their health assessment and physical examination, the diagnosis of their condition and its management or referral to another member of the multi-disciplinary team deemed appropriate by the practitioner within their professional competencies.
Make differential diagnosis using decision making and problem solving skills.
Make professionally autonomous decisions for which they are accountable.
Order necessary investigations and provides treatment and care, both individually, as part of a team and through direct referral to other primary and secondary agencies.
Plan, implement and evaluate the chronic disease management programmes in line with current evidence based best practice and the GMS contract, reducing unnecessary hospital admissions and promote the preventative aspect of nursing practice.
Prescribe appropriately as an Independent Nurse Prescriber, using the practice formulary
Screens patients for disease risk factors and early signs of illness following NICE guidelines.
Create patient care plans to include health promotion, health protection and disease prevention as appropriate.
To record information in patient records accurately and in line with current professional guidelines.
Management
Work to the NMC Code of Professional Conduct.
Provide leadership and support to the clinical team at the practice.
Actively monitor progress in the delivery of contracted services.
Make a significant contribution to the development and direction of the practice.
Participate in multi-disciplinary meetings.
Participate in staff appraisal.
Promote communication and provide learning opportunities for other staff.
Education, Training and Development
Hold a Masters degree in Advanced Practice
Develop new skills by undertaking training as required.
Maintain current skills by study and attending update training.
To maintain a professional portfolio and actively keep up to date with the latest developments and guidelines for best practice and disseminate this information accordingly.
Take part in the appraisal process.
Revalidate in line with national guidelines.
Encourage, facilitate and participate in clinical supervision within the practice.
To be a resource for advice and guidance for other members of the team.
Research
Monitor research in order to improve quality care and disseminate research finding.
Ensure evidence based practice personally and throughout the clinical team.
Undertake an active role in research and audit to better improve patient outcomes.
