The Social Prescribing Link Worker will operate within the Rotherham Central North Primary Care Network, specifically at Broom Lane Medical Centre and other associated practices. Two positions are available. The primary purpose of the role is to help patients identified by GP practices access support from local voluntary and community sector (VCS) organizations and statutory services to enhance their health and well-being.

Key Responsibilities:

  1. Referrals: Promote social prescribing, build relationships with GP practices, and encourage self-referrals.
  2. Personalized Support: Provide one-on-one support, co-producing personalized plans that address individuals' health and social needs, and assist in connecting them to community resources.
  3. Community Support: Foster strong connections with local VCS organizations to receive referrals and ensure their capacity to support vulnerable individuals safely.
  4. Collaboration: Work with local partners to strengthen community groups, encouraging volunteerism and peer support among former recipients of social prescribing.

General Duties:

  • Engage in ongoing professional development and supervision.
  • Comply with organization policies related to confidentiality, data protection, and safeguarding.
