The Health & Justice Youth Practitioner role with the Midlands Partnership NHS Foundation Trust in Crewe focuses on supporting children and young people navigating the Criminal Justice System. The position is part of a pilot program aimed at enhancing the Liaison & Diversion (L&D) services specifically for young people who face mental health issues, learning disabilities, and other vulnerabilities. The practitioner will conduct holistic health assessments, create reports for courts, develop risk management plans, and collaborate closely with youth justice services to ensure that the individual needs of young people are met, thereby promoting their personal growth and reducing recidivism.

In addition to carrying out assessments and providing support, the role involves utilizing outcome measures to evaluate the effectiveness of the L&D services. The practitioner will engage in promoting positive risk-taking and fostering self-determination, while also maintaining comprehensive health records and documentation. Travel across the MPFT footprint is required for this position, which emphasises a trauma-informed approach to care and the importance of building trust and rapport with young individuals in the justice system.
