The Nursing Associate role in Chesterfield with Derbyshire Community Health Services NHSFT involves providing patient-centered care in the community, particularly within patients' homes. This position is suited for both experienced and newly qualified Nursing Associates who are passionate about community care. The successful candidate will join a friendly District Nursing team dedicated to delivering evidence-based nursing care, with opportunities for ongoing training, education, and collaborative learning.

Key responsibilities include delivering high-quality nursing care to prevent hospital admissions and support patient discharges, as well as assessing, planning, implementing, and evaluating individualized patient care. The Nursing Associate will also provide guidance to junior staff, assist in maintaining healthcare environments, and collaborate with Integrated Community Teams, GP surgeries, and local acute Trusts to ensure comprehensive patient support.

Provide high quality nursing care to our patients in our local community to keep them safe at home, prevent hospital admission and support hospital discharge home.

Assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way whilst maintaining the patient's privacy and dignity.

Provide advice and support to junior staff in the delivery of high quality care and actively support the Community Nursing team in the management of the health care environment.

Implement/refer to the relevant services as appropriate.

Work closely with the Integrated Community Teams/GP surgeries and local acute Trusts.
