Procare Community Services is seeking a skilled Registered Nurse to join their team as a Community Matron in Guildford, offering a £2,000 Golden Hello. The ideal candidate will have experience at a minimum band 6, advanced health assessment skills, and a background in managing complex, frail patients with long-term conditions. The role involves managing a caseload of high-risk individuals, coordinating care across multiple services, including primary and community care, acute hospitals, and social and mental health services.

As a Community Matron, you will work within the Community Coordination Centre, conducting clinical reviews, prioritizing urgent referrals, and collaborating with multidisciplinary teams to create proactive care plans aimed at reducing hospital admissions. Candidates must possess a full UK driving license, an independent right to work, and be willing to cover late shifts and weekends.
