Synergy Health PCN in Nottingham is seeking two experienced Social Prescribing Link Workers (SPLWs), one focused on young people and the other on adult mental health. The role involves empowering individuals to manage their health and wellbeing via referrals to community resources and services, taking a holistic approach tailored to each person's needs. Key responsibilities include providing personalized support, building trusting relationships, co-developing support plans, managing caseloads, and understanding when to refer individuals to other health professionals.
