The post holder will work as a complex case manager, without direct supervision. You will be involved in delivering integration, working with GP's, Social services and the third sector organisations.
You will be providing case management, facilitating early discharge from hospital and preventing unnecessary/unplanned hospital admissions where appropriate as well as supporting healthcare closer to home.
The service currently operates from 09.00 - 17.00hrs, Monday to Friday.The post holder will be expected to support their team, department and organisation to achieve the Trust's Values in their day to day work.The post holder will be a Registered Nurse, with at least 2 years community experience of managing a complex caseload of patients.You will be responsible for a caseload of patients with complex health and social care needs. You will work autonomously to case manage these clients and use advanced nursing skills, such as assessment and prescribing to manage their conditions in the community.
They will work as part of the integrated care team, working within and across professional and organisational boundaries. Liaising with GPs, other professionals and statutory and voluntary agencies he/she will deliver individualised care to patients.
