We are pleased to invite applications from experienced Nurses to join a well-established team as a Community Clinical Nurse Specialist Palliative Care.
The main purpose of this role is to provide direct assessment, support and advocacy to patients and carers dealing with complex, distressing and life limiting illnesses.
You will assess referrals for admission to the service and perform comprehensive assessment of patients with sensitive and complex palliative care needs, as well as providing holistic specialist care for patients appropriate to their needs, wishes and culture.
This will include clinical assessment, planning care, monitoring progress, requesting appropriate investigations, and acting on the results.
