The Specialist Ageing Well Practitioner role at Lewisham and Greenwich NHS Trust involves delivering comprehensive care and support to older adults within the community as part of the Proactive Ageing Well Service (PAWS) over an 18-month fixed-term contract. The practitioner is responsible for identifying older adults who would benefit from Comprehensive Geriatric Assessments (CGA), managing patient caseloads, developing advance care plans, and coordinating care with a multidisciplinary team. The goal is to enhance the quality of life for older adults and prevent unnecessary hospital admissions by providing proactive, preventative, and patient-centered care.

Additionally, the role includes conducting medication reviews, improving communication with patients and caregivers, leading clinical development initiatives for staff, and participating in service improvement projects. The practitioner will also engage in research and audit activities, ensuring that best practices are upheld and that patient care pathways are optimized. Effective collaboration with other clinical areas and adherence to trust policies are critical components of this position, which aims to provide seamless and high-quality care for older adults in the community.
