The ANP will be responsible for treating minor illnesses and injuries, prescribing and reviewing medications, and contributing to service development and quality improvement initiatives to enhance patient care. This role requires an experienced professional capable of assessing, diagnosing, planning, and implementing care while collaborating closely with local practices and healthcare providers. Heartbeat Primary Care CIC have recently formed a General Practice Urgent Care Hub and are looking for an ANP to support facilitate the contract. The successful ANP will work across three local practices in the Hambleton South PCN area. The hours will be 12 per week spread across 3 days, days to be agreed. This is a fixed term contract for 1 year.

To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team. The duties will include all tasks normally undertaken by an experienced ANP and in addition any roles agreed between Heartbeat Primary Care CIC and local doctors as appropriate, having regard to current training.

Job Description:
- Conduct both face-to-face consultations in clinics, and telephone consultations.
- See patients of all ages
- Treat minor illness and/or Injury trained
- Prescribe and review medication, ensuring adherence to treatment plans.
- Manage cases autonomously
- Play a key role in service development and quality improvement initiatives to enhance care delivery.
