Hospital at Home Frailty ward have an exciting opportunity for a practitioner with advanced skills to join the team.

Hospital at Home- enabling people to receive acute care and treatments in home surroundings with support from a team of health and care professionals.

To be responsible for the rapid assessment, diagnostics, and treatment of individuals within the Hospital at Home service using an advanced level of professional accountability, autonomy, and judgement, underpinned by Masters level theory and experience. This includes highly complex decision making across a broad range of differentiated and undifferentiated individual presentations and complex situations, synthesising information from multiple sources to make appropriate, evidence based judgements and diagnoses,

To plan and manage complete episodes of care. This will be working in collaboration with senior medical colleagues and others when required. Delegating and referring as appropriate, to ensure timely, effective management to optimise health outcomes, in line with the evidence base and nationally recognised best practice.

To play a key operational role and maintain a pro-active approach to managing, updating and leading clinical pathways.

To promote and implement service and policy development and redesign, impacting beyond own area and other disciplines, informed by current best practice and evidence base, in accordance with values-based care

Are you a compassionate Registered professional with advanced clinical skills seeking an opportunity to work within a hospital at home team and understand what the service can offer patient within somerset in order to keep the patient within own home environment.

Our service covers the urban and rural areas of Somerset, predominantly based in one area however cross cover especially at weekends will be required.

Hospital at Home operate between 8am and 8pm we offer a variety of shift within our rota.

We offer flexible working solutions where possible to ensure a work-life balance . Weekend cover is required as part of the role. 30 hours is minimal contract hours for this role.

Advanced skills such as non- medical prescribing are essential for this role.

You'll need to be a car driver with your own vehicle to visit patients. We offer a generous mileage allowance to cover the use of your car for NHS business, which includes both mileage and depreciation.

Attend MDT and huddles to discuss patient caseload and plan intervention.

Carry out autonomous assessment and treatment plans for patient within the caseload , manage expectation of patients.

Open and honest communication with patients, family and stakeholders.

Prioritise own time and support team to ensure caseload is managed.

Build relationship with neighbourhood teams to support patient to remain within own home.

Work with Internal and external teams to promote Hospital at home.

Participle in service development , identify own needs and support training to team and other teams in directorate.

Ensure as a ACP your meeting the 4 pillars of the role.
