Hepatobiliary Medicine at the Royal Free Hospital
The Royal Free Hospital has, for a long time been synonymous with research and clinical excellence in the field of hepatology, and with the current near exponential increase in the incidence of liver disease, we aim to remain at the frontier of patient care, research and education.
The Royal Free provides secondary and tertiary Hepatology services. There is excellent access to ITU, a 24 hour TIPS rota and 24 hour consultant led GI bleeding rota. The liver transplant service at the Royal Free has been established for 30 years. The number of transplants performed has increased recently and currently stands at approximately 140 liver transplants per year. We have a live donor program. We are a tertiary centre for patients with hepatobiliary and pancreatic malignancy and the full range of surgical, locoregional and oncological treatments is provided for these patients. Dedicated clinics are established for viral hepatitis, liver metabolic disease, primary hepatocellular malignancy, autoimmune liver disease, PBC, PSC, NAFLD, Vascular liver disease as well as clinics for general hepatology and liver transplantation. A number of very active clinical research programs run with themes such as clinical and experimental portal hypertension, acute and acute on chronic liver failure, liver support and liver transplantation, non-invasive fibrosis assessment, clinical trials of new anti-viral agents, metabolic studies of hepatic encephalopathy, pruritis pathways and examining the effect of remote ischaemia pre-conditioning on liver graft function.

A training contract and areas of particular interest would be identified prior to commencement of the post but the following areas could be provided at UHBW:
- Management of complex viral liver disease
- Decompensated chronic liver disease and alcoholic hepatitis
- Portal hypertension
- Liver cancer
- Long term follow-up of the transplant patient and transplant assessment
- Acute liver failure
- Complex biliary and autoimmune liver diseases
- Other training experience available: ERCP/EUS, research meetings, audit/governance
The trainee would have supervised clinic experience, through between 2 and 3 specialist Hepatology clinics weekly (including independently-run Hot-clinic for urgent referrals). Direct consultation with the supervising consultant would be expected at the end of each clinic; a half hour discussion session will be included. The trainee would participate in ward cover of Hepatology patients in conjunction with a consultant Hepatologist (there is always hepatology cover) and share responsibility for daily senior hepatology in-reach to AMU/ED/ITU and additional referrals.
It is expected that the trainee will present regular case summary and literature review (i.e. case-based) on a regular basis at the weekly Friday lunchtime Hepatology meeting. A log of these presentations will be kept and form part of the assessment of completeness of training.
