The Community Staff Nurse role in Talgarth with Powys Teaching Health Board involves providing holistic patient care through assessment, development, implementation, and evaluation of evidence-based practices. Key responsibilities include delivering safe, autonomous nursing care in patients' homes, including palliative care, long-term condition support, self-management promotion, and wound care. The nurse will collaborate with patients, carers, and a multidisciplinary team to ensure high-quality care, and support the community nurse team leader. Candidates must be Registered Nurses, with the ability to speak Welsh being desirable. English and Welsh speakers are encouraged to apply.
