The Midlands Partnership NHS Foundation Trust is seeking two Band 4 Care Act Assessors in Lichfield and Tamworth to focus on completing annual reviews within the Adult Social Care Teams. These permanent positions are suitable for individuals at the beginning of their social care careers, offering comprehensive support for skill and experience development. The assessors will collaborate with social workers and senior assessors to ensure safe and appropriate care that promotes independence, while engaging with service users, their families, and partner agencies to execute needs assessments, care planning, and reviews in line with the Care Act (2014) and Mental Capacity Act (2005).

Candidates should possess strong communication, time management, and organisational skills, along with proficient computer abilities. While knowledge of community support services is beneficial, it is not mandatory as training will be provided. Successful applicants must be flexible team players willing to travel across Staffordshire and have a clean driving license and vehicle for work purposes.
