The Royal Devon University Healthcare NHS Foundation Trust is seeking a Medicines Management Technician to join its Urgent Community Response (UCR) Team based at Crediton Hospital. This community-focused role involves working collaboratively with a multidisciplinary team to provide short-term support for patients at home, particularly those recently discharged from hospitals or at risk of admission. Key responsibilities include conducting medicines reconciliations, providing patient education, advising on medication adherence, and supporting the clinical decision-making of GPs and pharmacy colleagues.

The ideal candidate will be GPhC registered with excellent communication skills and a proactive approach to solving medication-related queries, ensuring patients optimize their use of medicines.
