The Care Coordinator position at Mellock Lane with the Cheshire and Wirral Partnership NHS Foundation Trust offers a part-time role (15 hours per week) within the Neston and Willaston Care Community Team. The successful candidate will serve as a key point of coordination for multi-disciplinary activities, collaborating with local authorities, GPs, and voluntary sector partners to ensure high-quality support for patients. Responsibilities include managing daily multi-disciplinary meetings, liaising with team members, and signposting patients and caregivers to relevant services.
