Are you looking for the flexibility of weekend work that can work around your other commitments? Primary Care 24 (PC24) are excited to share the position of Urgent Care Practitioner (UCP).
This could mean a blend of in hours and out of hours working to ensure you achieve a comfortable work life balance. If you have studied Clinical examination and diagnostics and you have experience of working in an Urgent Care, Primary Care or similar setting then we are looking to hear from you.
We offer innovative models for telephone triage and telephone consultations based in our modern contact centre, working as part of an MDT - including experienced GPs, GPs in training, Advanced clinical practitioners from a Nursing, Paramedic, AHP and Pharmacy background. Providing initial clinical assessment / triage for patients entering the service from NHS111 / Clinical assessment service (CAS), NWAS, healthcare professionals and primary care streaming. The primary aim of optimising their care navigation, which may include, selfcare, signposting towards other services / partner agencies or continuing onto the clinicians within PrimaryCare24 for further review.
Hours available:
- Fri 18.30-23.00
- Sat 08.00-13.00, 13.00-18.00, 18.00-23.00
- Sun 08.00 - 13.00, 13.00-18.00, 18.00-23.00
About us

- Manage clinical demands in collaboration with Clinical Co-Ordinator/ Shift Manager to ensure delivery of safe care to agreed waiting times
- Escalate concerns/ priority patients to the Clinical Co-Ordinator/ On call teams
- Review patients awaiting contact to provide oversight and minimise clinical risk by actively filtering for potential signposting to more appropriate services
- Based around a Red Flag approach of triage, prioritise patients in line with the service escalation plans which include the elderly, palliative patients and those who have a risk of deterioration
- Apply clinical skills and reasoning to ensure patients are transferred safely between different clinical dispositions and signposted when required and safe clinically safe
- Assess an individuals health status. Provide skilled and effective assessment of patients presenting clinical need, through telephone consultation, using professional clinical judgement with the support of clinically based algorithms
About you
- Extensive experience in emergency health care settings
- Significant expertise in clinical triage
- Degree Clinical Practice
- Minimum 3 years post qualification experience
- Clinical Examination and Diagnostics Level 6/7
- Experience of working in paediatrics (aged 2 yrs onwards)
- Experience of working in urgent care / primary care or similar setting
