Cambridgeshire and Peterborough NHS Foundation Trust is seeking a full-time permanent Community Mental Health Practitioner specializing in forensic care. The role involves working within a multi-disciplinary team to provide specialist assessment, care planning, and interventions for adults with mental health conditions or learning disabilities who pose a risk of harm to others. Practitioners will lead care planning processes, coordinate complex cases, and ensure effective collaboration with agencies to promote recovery and minimize re-offending. The position requires independent travel throughout the region, necessitating a valid UK driving license and access to a vehicle.

The service operates seven days a week and is dedicated to creating a seamless care pathway from inpatient units to the community. Practitioners are expected to adopt a trauma-informed approach, contributing to the evaluation and management of individual care needs while adhering to the Care Programme Approach and maintaining high standards of care. Participation in team meetings and collaboration with various healthcare professionals will be essential to ensure the delivery of quality care and support for service users.
