To support the Complex Discharge Team in facilitating patient flow and effective discharge.To prepare regular and detailed reports on medically optimised and delayed discharges for senior managers, consultants and as required, project board meetings and other senior level discussions.To support the Complex Discharge Coordinators and the MDT in facilitating safe discharges. To undertake urgent discharge activities as required.
