The Confederation, Hillingdon CIC is seeking a motivated Clinical Pharmacist for a six-month fixed-term position in the Hayes & Harlington Primary Care Network (PCN) to cover maternity leave. The successful candidate will collaborate closely with GPs across multiple practices, participating in a multidisciplinary team to enhance medication management, including the review of repeat prescriptions and the optimisation of patient care for chronic conditions such as respiratory, cardiovascular issues, and diabetes. Key responsibilities include conducting clinical medication reviews, managing common ailments, ensuring safe medicine practices, and providing education and support to both patients and healthcare staff.

Candidates should hold a pharmacy degree and be registered with the General Pharmaceutical Council, with a minimum of two years' pharmacy experience. While an independent prescribing qualification is preferred, it is not mandatory. The role requires strong organisational and problem-solving skills, with a focus on patient involvement in medication decisions and adherence to best practices. The Clinical Pharmacist will also play a vital role in service development, quality improvement, and collaboration with various healthcare providers to enhance patient outcomes and safety in medication management.
