The Dr Raut Partnership in Hull is seeking a part-time (16 hours) Mental Health Support Worker SPLW to provide non-clinical support for patients experiencing low-level mental health concerns. The ideal candidate should have experience working with vulnerable individuals and possess the ability to empathize, intervene, and direct patients to appropriate services. This role involves gathering referrals, developing personalized support plans, and fostering relationships with individuals and their families to promote independent living and improved health outcomes.

As a member of the primary care network multi-disciplinary team, the Mental Health Support Worker will address broader health determinants such as debt, housing, and community engagement. Responsibilities include managing caseloads, recognizing emergencies, collaborating with local agencies, and educating both clinical and non-clinical staff about available community resources. The role emphasises a holistic approach to mental health, with adherence to organisational policies and regular supervision to handle complex patient needs.
