To work with clinical staff within the acute hospitals to support and ensure the provision of safe, high quality services for individuals with learning disabilities using inpatient services, minimising risk, incident and error and fostering a culture of learning and reflection.

To support implementation of the Directed Enhanced Service (DES) Annual Health checks for people with Learning Disabilities, through provision of specialist education and support to Primary Care Staff to ensure that practices are able to meet its requirements and to promote uptake of the DES amongst practices across West Sussex.

A band 6 acute liaison nurse/health facilitator job has become available in the Learning Disability Health Facilitation Team. Other qualified professionalswould also be considered with appropriate learning disability experience.

The Learning Disability Health Facilitation Team support people with learning disabilities to access health care (in hospitals and in the community) within West Sussex. The successful applicant would be based in the Princess Royal Hospital and covering area's including Haywards Heath and Burgess Hill health facilitation work.

To provide specialist learning disability knowledge, expertise and input to support access to healthcare for people with a learning disability. To ensure that people with a learning disability living in West Sussex can effectively access primary and secondary health care services as required, resulting in improved health outcomes.

Kindly note that this is an essential qualification for this job:

Registered Nurse Qualification or allied health professional and current entry on a professional register

Evidence of ongoing training in specialist area of learning disabilities and commitment to development within qualified profession
