This post is to support the Virtual Ward model which is a collaboration with the acute trust to deliver hospital standard treatment within a patients' home. The virtual ward philosophy is that care is led by a specialist and provides acute clinical care at home which would otherwise be provided in a hospital for a short duration of up to 14 days. This community service is nurse led with access to support at the Virtual monitoring Hub.
This service is part of the Urgent Care Services which provides a two-hour response to patients in crisis at home. Our aim is to preventunnecessary hospital admission and facilitate early discharge from the acute hospitals. We also provide unscheduled care out-of-hoursfor patients who may be at the end of life, or experiencing an urgent need for healthcare. This may include treating acutely unwell peoplethat would otherwise require hospital admission, and also supports hospital discharges. The service convers 24 hours a day 365 days ayear.Nurses use a wide clinical skills base to provide hospital-level care at home. This includes IV therapy, end of life symptom control andmanagement via syringe driver, catheterisation skills, phlebotomy, wound management, risk assessment, as well as general observationsand fundamental nursing care.This service is providing nursing care within patients' homes and covering West Kent. The ability to drive is essential to this post.
