Hospice in the Weald would love you to join our dynamic hospice team.

We are seeking an experienced and passionate Advanced Clinical Practitioner in Palliative Care to provide high-quality, patient centred care.

You will work at our Cottage Hospice andour Pembury inpatient wards. You will have or be working towards and MSC in Advanced Practice and be anon medicalprescriber with a physical assessments qualification. You will have advanced communications skills, clinical assessment anddiagnosis and decisions skills within a variety of general and palliative diagnoses.

You will work closely with our medical and multidisciplinary teams and be part of the 1st on call rota on an ad hoc basis.

About us

Deliverables/outcomes

Directly manage care of patients and those important to them with complex Hospice Palliative Care needs.

Undertake complex and sensitive holistic assessments, plan, implement and evaluate care utilising advanced communication, clinical reasoning, diagnostic and prescribing skills.

Diagnose and manage both acute and chronic palliative care conditions integrating both pharmacological and non-pharmacological based treatments into a clinical management plan.

Manage patient level communications with other health and social care professionals and agencies to promote planned, coordinated, seamless care.

Provide expert advice on symptom control, advance care planning, social and psychological care, bereavement, communications and use of resources.

Improve access to medication, especially anticipatory medications, using non-medical prescribing, remote prescribing and eWorks

Prescribe and review medication for therapeutic effectiveness and appropriate to the patient in accordance with evidence- based practice including national and local protocols and clinical guidelines within the Scope of Practice

Prioritise the Hospice Palliative Care health problems and intervene appropriately to assist the patient in complex, urgent or palliative care emergency situations, including initiation of effective emergency care.

Be a link between the wards and the Outreach service ensuring that patients transition seamlessly between the two

Act as Outreach Clinical lead of the day on rotation.

Management & Clinical Leadership

Meet regularly with the Clinical Director and other Hospice ACPs sharing experiences and ways of working, shaping the role, and looking to constantly improve.

Line manages lower grade staff within the Hospice Outreach service or Cottage Hospice

Works autonomously at the Cottage Hospice (ACP led service).

Contribute to the learning of the more junior staff formally, and by role modelling in effecting robust clinical interventions and making authoritative decisions.

Make a significant contribution to the development of Hospice services and the wider Hospice clinical offering.

Take an active role in the Clinical Sub-Group of the Hospice Leadership Team or other Hospice meetings as directed by your manager.

Contribute towards the clinical governance process including the reporting of significant incidents/near misses, analysing activity data and investigating incidents or complaints as directed.

Take an active part in the review, writing and publication of patient and those important to them literature and clinical policies and guidelines.

Ensure identification of patients with an IPOS of 3 or 4 are discussed at each MDT role modelling an effective use of MDT

Communications and Teamwork

Collaborate with health care professionals, relevant agencies across the neighbouring Trusts and Social Service Departments in order to promote planned, coordinated patient care.

Lead and influence health care professionals about the importance of early identification and management of End of Life Care.

Model Advanced Communication Skills in interactions with patients, families and carers.

Advance the use of End of Life Care Tools and outcome measures such as IPOS, POI and RESPECT, modelling their use and supporting others to use them effectively.

Audit and Research

Provide clinical expertise to guide the design and delivery of research projects undertaken at HitW.

Promote and foster an environment where evidence-based practice is grounded in high quality research and audits.

Conduct and/or support interdisciplinary audits, identifying and implementing improvements for clinical practice.

Support research data collection activities at HitW. Ensure Good Clinical Practice (GCP) and other relevant qualifications to the research activity are up to date.

Aim to author or co-author conference publications/poster or papers that are published in reputable peer-reviewed journal.

Maintain competence and up-to-date knowledge on developing areas in Hospice Palliative Care by attending and participating in journal clubs, research symposiums, seminars, and/or conferences

Participate in Hospice wide initiatives which facilitate quality via subgroups of HLT.

Participate in the development of clinical standards within the Clinical Governance framework of the Hospice.

Training and Development

Work closely with the Outreach Service and staff on both wards to model excellent advanced communication and proactive care planning. Offer training and side by side working as needed to improve the skills of staff.

Develop clinical expertise in the Clinical Nurse / Paramedic Specialists (CNS/ CPS) to be expert assessors and advisors on symptom control and the utilisation of advanced clinical reasoning and communication skills.

Recognise the individual learning needs of colleagues, patients and those important to them and the most effective way of addressing them.

Actively contribute to the development and delivery of any Hospice learning opportunities.

Create a progressive learning environment, encouraging all staff to learn from each other.

Facilitate the professional development of staff in the Outreach service to enable them to develop enhanced palliative care knowledge and skills through 1:1 and team learning opportunities.

Follow the royal pharmaceutical guidelines (2and competencies by meeting with the NMP clinical lead monthly, undertaking audits and developing NMP responsibilities.

Undertake Mandatory Training as required by Hospice in the Weald and participate in additional required training and development activities.

Take an active role in identifying gaps in knowledge and skills and how to address these.

Take responsibility for setting own goals and achieving them.

General

Maintain professionalism through adherence to recognised codes and guidelines and work within the boundaries of Hospice in the Weald policies.

Maintain personal development through the Hospice Annual Review process, continuously updating and improving knowledge and skills and applying that knowledge.

Engage in the use of information and communication technology (ICT) and achieve the required level of competence.
