Devon Partnership NHS Trust is looking for a dedicated and motivated Band 6 Senior Community Mental Health Practitioner to join the East & Mid Devon Home Treatment Team based at Wonford House. The role involves providing home treatment to clients with complex mental health needs, striving to avoid hospital admissions and facilitating early discharges when necessary. The successful candidate will work as part of a dynamic multi-disciplinary team, developing recovery-focused care plans and strategies for clients, while also coordinating with various agencies to ensure high-quality support for clients and their families.

Candidates should possess a professional mental health qualification, such as RMN, CQSW, or equivalent, and hold current registration with a relevant professional body. The position requires strong communication skills and the ability to work independently, managing care for vulnerable individuals with diverse and ongoing needs. The team is committed to making a meaningful difference in people's lives by creating personalized treatment packages that inspire hope and recovery.
