The Care Coordinator position within the Colchester Specialist Community Mental Health Team at Essex Partnership University NHS Foundation Trust involves assessing and coordinating care for a designated caseload of service users. Key responsibilities include evaluating new referrals, creating and updating person-centred care plans, managing risks, and facilitating access to additional services such as crisis teams or hospital admissions when necessary. The role also includes administering long-acting anti-psychotic injections and monitoring medication adherence, while ensuring compliance with the Mental Health Act 1983 and identifying safeguarding concerns.
