HCRG Care Group is seeking a Bank Health Visitor to join their Healthy Family Team in central Lancashire, including Chorley, Preston, South Ribble, and West Lancashire. This flexible position is ideal for Health Visitors looking for weekend work or additional hours outside the traditional Monday-Friday schedule.
Key Responsibilities:
- Deliver the Healthy Child Programme, assessing health needs and creating care packages.
- Use the Family Health Needs Assessment tool for planning and implementing care.
- Develop individualized care plans for children aged 0-5, involving parents and families in the process.
- Assess health, growth, and development, utilizing the Early Help Assessment Tool when necessary.
- Collaborate with partner agencies, taking a lead role when appropriate.

SYNOPSIS • FULL DESCRIPTION
There is a Healthy Family Team located within a range of districts across central Lancashire and is made up of a multi-disciplinary team, including Health Visitors, School Nurses and additional colleagues who work together to provide family support.
As a
Bank Health Visitor,
you will be responsible for:
- The delivery of the Healthy Child Programme including assessment of health needs, development, implementation and evaluation of packages of care
- Using the Family Health Needs Assessment tool to plan and evidence care
- Devise and implement individualised care plans for each child based on a needs assessment, within the Health Visiting Strategy, while actively involving the child/parent/carer/family in the planning and implementation of any proposed programme of care and support
- Identify and assess health, growth and development in the 0-5 age range (0-19 Pathway)
- Use the Early Help Assessment Tool when there is unmet need identified.
- Participate in effective partnership working with other agencies and taking a lead professional role where appropriate.
- Use evidence-based practice to develop and maintain a high quality and cost-effective standard for Specialist Public Health Nurse/Health Visiting practice.
