HCRG Care Group is seeking a Bank Health Visitor to join their Healthy Family Team in central Lancashire, including Chorley, Preston, South Ribble, and West Lancashire. This flexible position is ideal for Health Visitors looking for weekend work or additional hours outside the traditional Monday-Friday schedule.


Key Responsibilities:

  • Deliver the Healthy Child Programme, assessing health needs and creating care packages.
  • Use the Family Health Needs Assessment tool for planning and implementing care.
  • Develop individualized care plans for children aged 0-5, involving parents and families in the process.
  • Assess health, growth, and development, utilizing the Early Help Assessment Tool when necessary.
  • Collaborate with partner agencies, taking a lead role when appropriate.
  • Maintain high standards of practice based on evidence.
